What is the Shelter Specific Model? (SSM)
Every community has different needs that require specific solutions and consistency. This is why each of our partner organizations has a dedicated team that curate event programming based on patient testimonials and needs assessments. We also recognize that our mission -much like health- cannot be achieved overnight; this is why our teams visit our shelter partners around 8 times a year!
Through the SSM, patients will:
See the same team members at each event, so YOU can get to know us better
Receive local resources that are nearby and accessible to you
Offer feedback for future events and see the feedback in action
Continue to monitor progress on health goals with our team and our attending physicians
